About the Position
HomeWell Care Services is seeking an experienced and compassionate Client Care Manager to help ensure our clients receive dependable, personalized, high-quality care.
This is a non-medical care-management position—not a traditional direct-care caregiving role.
You will work closely with clients, families, caregivers, and our office team to conduct assessments, monitor care quality, support caregivers, communicate with families, and resolve concerns.
A nursing license is not required. We welcome experienced Care Coordinators, Lead Caregivers, Caregiver Supervisors, CNAs, and other senior-care professionals ready to grow into a care-management leadership role.
Key Responsibilities
- Conduct in-home client assessments and reassessments
- Develop and update individualized Plans of Care
- Conduct supervisory and quality-assurance visits
- Follow up with clients and families after new starts of care
- Monitor client satisfaction and identify concerns
- Recognize and report changes in client condition, safety, mobility, behavior, or care needs
- Help match caregivers with clients based on skills, experience, and personality
- Coach caregivers on Plans of Care, client expectations, professionalism, and performance
- Address caregiver/client compatibility and service-quality concerns
- Communicate with clients and families regarding care needs and service changes
- Collaborate with the Scheduler/Care Coordinator to resolve service issues
- Maintain accurate and timely documentation
- Follow concerns through resolution and escalate significant issues appropriately
Qualifications
- 2+ years of experience in home care, senior care, caregiving, care coordination, senior living, hospice, home health, or a related setting preferred
- Experience working with older adults
- Dementia/Alzheimer's experience strongly preferred
- Care coordination, caregiver supervision, client services, scheduling, assessment, or leadership experience is a plus
- Strong communication, organization, documentation, and problem-solving skills
- Comfortable coaching caregivers and communicating with families
- Able to work independently and follow through on concerns
- Valid California driver's license, reliable transportation, and ability to travel throughout the service area
- Comfortable using computers and electronic documentation systems
CNA or other healthcare credentials are welcome but not required.
Schedule & Compensation
Approximately 20–25 hours per week, primarily weekdays, with flexibility for assessments, supervisory visits, new starts of care, and occasional time-sensitive client needs.
$28–$33 per hour, depending on relevant experience and qualifications.
Mileage reimbursement is provided for authorized business travel.
As our client base grows, there may be opportunities for increased hours and additional leadership responsibilities.
