Executive Director, RN

Our client is a leading provider of homecare and home health services in Philadelphia. Thier mission is to provide exceptional services to patients who need our help the most.

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Supervise team of individuals who provide hospice care to our patients
• Assess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s hospice needs
• Counsel patient and patient’s family on end-of-life needs
• Manage emergency situations in a prompt, precise, and professional manner
• Complete medication administration and treatment needs for all assigned hospice patients
• Provide nursing care in accordance with applicable regulations and Company policy
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

• Active RN state license
• Valid drivers’ license
• Experience working in the hospice field
• Two or more years of applicable nursing experience (supervisory/leadership experience preferred)
