    Major Duties

    Serves as the installation non-clinical Case Worker for the USMC Exceptional Family Member Program (EFMP). Manages services including intake, identification, assessment, evaluation, tracking, and referral of eligible family members. Acts as a liaison with medical, non-medical, installation, and community professionals to ensure top-tier care aligned with national and Marine Corps standards.

    Core Responsibilities :

    • Case Management: Conducts assessments, completes Family Needs Assessments and Services Plans, and establishes/maintains electronic case files with detailed contact notes in the EFMP Case Management System (CMS).

    • Family Support: Contacts each EFMP family upon assignment notification. Routinely assesses circumstances, assists families in creating and achieving goals, and empowers them to pursue available resources.

    • Resource Navigation: Connects families to vital services including Respite Care, Special Education, Supplemental Security Income/Social Security Disability Insurance, Medicaid Waivers, Early Intervention, and Tricare.

    • Advocacy & Collaboration: Participates in Individualized Education Plan (IEP) and Inclusion Action Team (IAT) meetings per MCO 1754.4. Contributes to a library of special needs educational and referral materials.

    • PCS & Transition Assistance: Provides "warm hand-off" assistance during Permanent Change of Station (PCS) relocations. Coordinates with losing/gaining case workers and advises families on relocation options (e.g., assignment reconsideration, Continuation of Location).

    Compliance, Safety, & Administrative Duties :

    • Information Security: Safeguards highly sensitive, proprietary, and privileged data. Ensures strict compliance with the Privacy Act (5 USC 552a), HIPAA (PL 104-191), and DoD Directive 6025.18. Restricts unauthorized access or dissemination and reports data releases immediately.

    • Reporting & Engagement: Keeps the EFMP Manager updated on caseload status, acute cases, and program compliance. Attends Special Needs Forums and collaborative events at the Managers discretion.

    • Customer Service & Workplace Standards: Delivers exceptional, professional customer service. Promotes Equal Employment Opportunity (EEO) principles and the prevention of sexual harassment. Adheres to safety regulations and promptly reports workplace hazards or mishaps.

    Physical & Schedule Requirements :

    • Schedule: Requires occasional evening and weekend hours to support family engagement activities and support groups.

    • Physical Demands: White-collar position requiring occasional lifting of up to 20 lbs.

    Qualifications

    Bachelor¿s degree in behavioral health or a related social science, or combination of equivalent education and experience, or four years of appropriate experience that demonstrates knowledge of one or more of the behavioral or social sciences and working in a case management capacity with families who have members with special needs. Case Management certification from an accredited source preferred. Knowledge of the Military lifestyle to include service/unit organization and rank structure and military, family and personal support services available.

    Knowledge of military laws, policies, regulations, resources and practices regarding medical and educational benefits, programs, services and community agencies for family members enrolled in EFMP. Skills in active and empathic listening and interviewing and applying sound judgment for assessing individual and family needs; problem-solving and advocating for support or services; or determining an appropriate course of action. Written and oral communication skills to effectively provide case management assistance to people in various emotional states and varying needs; and to conduct briefs, workshops and training. Inter-personal skills to relate to all segments of the military and civilian community using persuasiveness, poise, tact and objectivity. Proficiency with computer software and applications involving word processing, email communication, database management desktop publishing, and internet navigation.
