Licensed Practical Nurse (LPN) Evening Shift
Connected Health is seeking a dedicated Licensed Practical Nurse (LPN) to join one of our trusted partner facilities in Ishpeming, Michigan. This 140-bed skilled nursing and long-term care facility is committed to delivering exceptional resident care in a supportive and collaborative environment.
Position Details:
- Schedule: 3:00 PM 11:00 PM
- Shift: Evening
- Hours: 5x8 schedule (40 hours per week)
- Location: Ishpeming, MI
Responsibilities:
- Administer medications and treatments as prescribed
- Monitor resident health status and report changes to the care team
- Maintain accurate and timely documentation
- Collaborate with physicians, nurses, and other healthcare professionals
- Provide compassionate, resident-centered care
- Support residents and families with professionalism and empathy
Qualifications:
- Current Licensed Practical Nurse (LPN) license in Michigan
- Skilled nursing or long-term care experience preferred
- Strong clinical, communication, and organizational skills
