Job Description:
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide quality nursing care to residents in a long-term care facility. The LPN will be responsible for delivering direct patient care, monitoring residents' health conditions, administering medications, and collaborating with the healthcare team to ensure residents receive safe and effective care. The role may require floating between two nursing units based on staffing needs.
Key Responsibilities
· Provide direct nursing care to residents in a long-term care setting.
· Administer medications and treatments in accordance with physician orders and facility policies.
· Monitor and document residents' vital signs, health conditions, and changes in medical status.
· Assist residents with daily living activities and support their overall comfort and well-being.
· Develop, implement, and update individualized care plans in collaboration with the healthcare team.
· Maintain accurate and timely documentation using Point Click Care (PCC) or similar electronic medical record systems.
· Communicate effectively with physicians, registered nurses, residents, and family members.
· Observe and report changes in residents' conditions and respond appropriately to medical concerns.
· Follow infection control procedures, safety guidelines, and established nursing protocols.
· Float between assigned nursing units as needed to support patient care and staffing requirements.
Qualifications
· Current and valid Licensed Practical Nurse (LPN) license.
· Previous experience in long-term care, skilled nursing, or geriatric care preferred.
· Knowledge of medication administration, nursing procedures, and resident care standards.
· Experience with electronic medical record systems; Point Click Care (PCC) experience preferred.
· Strong communication, organizational, and clinical assessment skills.
· Ability to work independently and collaboratively in a healthcare environment.
· Compassionate approach to resident care and commitment to patient safety.
