Job Summary<\/span>

<\/span><\/span><\/h2> We are seeking an experienced <\/span><\/span>Epic Healthy Planet with Campaigns Consultant<\/span><\/span><\/b> to support a large-scale healthcare technology initiative focused on population health management, patient engagement, and outreach optimization. This consultant will serve as a subject matter expert for Epic Healthy Planet and Campaigns, partnering with operational, clinical, and technical teams to design, implement, optimize, and support population health and patient outreach workflows.<\/span>

<\/span><\/span><\/p> The ideal candidate has extensive experience working with Epic Healthy Planet, Campaigns, and population health initiatives, with a strong understanding of care management, preventive care programs, patient engagement strategies, and reporting capabilities within Epic.<\/span>

<\/span><\/span><\/p> Assignment Details:<\/span><\/span><\/b>

<\/span><\/span><\/p> Initial 6-month contract with likelihood of extensions<\/span>

<\/span><\/span><\/li> Approximately 25% onsite as needed throughout the engagement<\/span>

<\/span><\/span><\/li> <\/ul> Key Responsibilities<\/span>

<\/span><\/span><\/h2> Serve as the primary Epic Healthy Planet and Campaigns subject matter expert.<\/span>

<\/span><\/span><\/li> Gather business, clinical, and operational requirements related to population health, care management, patient outreach, and quality initiatives.<\/span>

<\/span><\/span><\/li> Configure, optimize, and support Epic Healthy Planet and Campaigns functionality.<\/span>

<\/span><\/span><\/li> Design and implement patient outreach strategies using Campaigns and related Epic tools.<\/span>

<\/span><\/span><\/li> Collaborate with clinical, operational, analytics, and leadership teams to support population health initiatives and patient engagement programs.<\/span>

<\/span><\/span><\/li> Build, maintain, and optimize campaigns, registries, worklists, dashboards, and reporting solutions.<\/span>

<\/span><\/span><\/li> Support quality, preventative care, risk stratification, chronic disease management, and care gap closure initiatives.<\/span>

<\/span><\/span><\/li> Participate in workflow assessments and recommend system enhancements to improve operational efficiency and patient outcomes.<\/span>

<\/span><\/span><\/li> Support testing activities, including unit testing, integrated testing, user acceptance testing, and issue resolution.<\/span>

<\/span><\/span><\/li> Develop and maintain project documentation, build specifications, workflow diagrams, and training materials.<\/span>

<\/span><\/span><\/li> Provide go-live, stabilization, and post-implementation optimization support.<\/span>

<\/span><\/span><\/li> Partner with Epic application teams, reporting teams, and operational stakeholders to ensure successful project delivery.<\/span><\/span>

Requirements<\/h3> Required Qualifications<\/span>

<\/span><\/span><\/h2> Minimum of 5 years of Epic Healthy Planet experience.<\/span>

<\/span><\/span><\/li> Experience implementing, supporting, or optimizing Epic Healthy Planet and Campaigns.<\/span>

<\/span><\/span><\/li> Strong knowledge of population health management, patient engagement, care management, and preventative care workflows.<\/span>

<\/span><\/span><\/li> Experience gathering requirements, performing workflow analysis, and translating business needs into Epic solutions.<\/span>

<\/span><\/span><\/li> Experience supporting testing, validation, and issue resolution activities.<\/span>

<\/span><\/span><\/li> Strong analytical, troubleshooting, and problem-solving skills.<\/span>

<\/span><\/span><\/li> Excellent communication and stakeholder management abilities.<\/span>

<\/span><\/span><\/li> Experience working in complex healthcare environments.<\/span>

<\/span><\/span><\/li> <\/ul> Preferred Qualifications<\/span>

<\/span><\/span><\/h2> Epic Healthy Planet certification.<\/span>

<\/span><\/span><\/li> Epic Campaigns experience strongly preferred.<\/span>

<\/span><\/span><\/li> Experience supporting value-based care, quality measures, risk adjustment, or care management initiatives.<\/span>

<\/span><\/span><\/li> Experience with registries, dashboards, reporting, and population health analytics.<\/span>

<\/span><\/span><\/li> Prior experience supporting large healthcare organizations and enterprise Epic environments.<\/span>

<\/span><\/span><\/li> Consulting experience preferred.<\/span><\/span>
