    Description

    Upperline Health, the nation's largest provider group focused on lower extremity, wound, and vascular care, is hiring a Practice Success Coordinator to support our value-based care program, Upperline Plus.
     
    In this onsite, patient-facing role, you'll work daily at partner practices helping patients understand and enroll in Upperline Plus using an iPad. You'll build trust through friendly conversations, explain program benefits during check-in/check-out, and collaborate with clinic staff to hit enrollment and engagement goals and improve workflows.
     
    About This Role:
    You're educating patients and guiding them to enroll in a healthcare program, similar to a consultative sales or enrollment role. No sales experience is required, just strong interpersonal skills and the ability to clearly communicate value. Compensation includes a commission component tied to enrollment and engagement goals, making this a great fit for motivated, results-driven candidates looking to grow their sales skills in a healthcare setting. Our team enjoys excellent work/life balance - no night, weekend, or holiday shifts are required!
     
    What You'll Do:
    • Educate and enroll patients in the program
    • Help patients see the value of engaging in the program and guide them through scheduling visits
    • Build rapport with patients and clinic teams
    • Partner with clinic staff to troubleshoot issues and refine enrollment workflows
    • Represent Upperline's culture and values at partner sites
     
    What You Need:
    • High school diploma or equivalent
    • 2+ years of customer service experience
    • Comfort with iPads and Microsoft Office
    • Strong communication skills and HIPAA awareness
     
    Preferred:
    • Background in retail, hospitality, or call centers
    • Experience working with older adults/geriatric populations
    • Results-driven, mission-motivated mindset

Compensation is competitive, includes a commission structure tied to enrollment performance, and is based on regional standards and experience. Earn $16–$20/hour based on your experience — plus monthly commission that rewards you for the results you drive!
