The Position : We are looking for a full-time Enrollment/ Intake Registered Nurse (RN) to provide care to patient's via their individual care plans. The client offers the following compensation package:
$80,000- $103,000 Salary Based on Experience

R esponsibilities:
Monday- Friday
8:00am- 4:00pm
No Nights
No Weekends
Frequent Local Travel
Conduct s Assessment s to determine prospective patients' eligibility for the Pace Program.
See Patient in Their Home and the Centers
No On Call Responsibilities
Requirements:
MA RN Licensed
CPR Certified
Covid Vaccination not required. Flu shot required for all staff.
MDS Assessment Experience preferred ( Minimum Data Set)
Must Have Experience Working with the Geriatric Population
Build Strong Relationships With Participants
Must Have Great Communication Skills
