POSITION SUMMARYThe Engagement Specialist provides centralized engagement, outreach, and care coordination support to children and adults receiving behavioral health services. This role is responsible for improving patient access, retention, and continuity of care through proactive communication, follow-up, and coordination with clinical teams and referral sources.This position operates from a centralized location alongside the Care Management team, supporting multiple programs, health center sites, and service lines.
HOURLY RATE $20
EDUCATION AND EXPERIENCE/QUALIFICATIONSHigh School diploma required. Minimum of one year of progressively responsible work experience, preferably in a healthcare or behavioral health setting.Experience with electronic health records and Microsoft Office applications required.Strong organizational, communication, and coordination skills.Ability to manage multiple priorities and maintain accurate documentation in a fast-paced environment.

ESSENTIAL DUTIES AND RESPONSIBILITIESCollaborates with clinicians and care teams to support patient engagement and retention across individual and group-based behavioral health services.
Conducts proactive outreach to patients, including those new to services, recently referred, or difficult to engage.
Contacts patients who miss scheduled appointments and facilitates timely rescheduling.
Maintains ongoing communication with referral sources, collateral contacts, and primary care providers to support continuity of care.
Sends engagement and discharge communications in accordance with program protocols.
Assists patients in accessing recovery supports, housing resources, and other identified needs.
Tracks and manages engagement workflows, including outreach attempts, outcomes, and follow-up actions.
Produces and delivers timely, accurate, and professional documentation in the electronic health record and other systems.
Monitors assigned caseloads or worklists to ensure no gaps in outreach, follow-up, or engagement activities.
Maintains professionalism in all interactions with patients, staff, and external partners.
Meets established performance metrics and key performance indicators related to access, engagement, and retention.
Participates in ongoing training and staff development to enhance skills, efficiency, and knowledge of best practices.
Demonstrates cultural competency and applies inclusive, patient-centered engagement approaches.
Performs additional administrative and coordination duties as assigned.
