We were originally established in 1964 as a community-based alternative to institutional care and over the years have developed a broad array of services based on the principle that full inclusion and participation in community life is attainable for every individual with an intellectual or developmental disability. As an organization, we are committed to enriching the lives of people with I/DD by partnering to provide expertise, support and advocacy in their pursuit of a meaningful life.

    Case Management

Case Management is a collaborative processes of assessment, planning, facilitation, care coordination, evaluation and advocacy. Our Case Managers and Case Management Specialists take a person-centered approach in assisting those we serve and their families in living a more meaningful life. We strive to empower individuals to have control of their lives and make safe and informed choices.

    Community Outreach
