The Eligibility WIC Specialist Screens and assists with the application process Mountain Park Health Centers discount program, medical assistance programs available, and the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC).
Essential Functions
- Provides information on health care programs to patients and screens for appropriate medical coverage eligibility available through state programs and assists patients in application process for medical programs.
- Utilizes and abides by proper charting and documenting procedures for all patient related services, interactions, and assignments performed.
- Guides and assists patients in utilizing recommended services as indicated by medical referral and make appropriate referrals to community agencies on a case-by-case basis. Orients patients to care delivery system and available resources.
- Ensures income and other required information are correctly transferred into the Practice Management System and an appropriate discount plan is given to all eligible patients.
- Determines WIC program eligibility in accordance with local, state, and federal regulations and procedures.
- Conducts nutrition assessments, evaluates dietary intake patterns based on the dietary questionnaire.
- Provides participant centered nutrition education to participants individually or in groups according to age, category, health risks, and family needs.
- Assigns appropriate food package for eligible participants based on age, category, health risks and family needs.
- Provides breastfeeding support and promotes a culture of breastfeeding.
- Travels to multiple locations during shift.
- Maintains regular and predictable attendance.

POSITION QUALIFICATIONS
Minimum Qualifications:
- High School Diploma or general education degree (GED)
- Valid drivers license
Preferred Qualifications:
- Experience in, or related field
- Associate degree
