    Description

    PURPOSE

    The Clinical Practitioner II (EIS-FQ) will provide direct services for babies and toddlers, birth to age 36 months and complete the associated documentation for the consumer and program. The EIS-FQ is also qualified for a lateral position as an EIS Enroller within the ECI program.

    TASKS

    1. Provide Specialized Skills Training (SST) for babies, toddlers and families; developmental assessments and develops the IFSP treatment plan for child and family consumers in their natural environments 2. Ensure clinical documentation is submitted in accordance with Center standards, and meets all designated requirements 3. Link and refer families/caregivers to appropriate services (internal and/or external) as indicated 4. Comply with and participate in Quality Assurance and Quality Improvement processes 5. EIS-FQ professionals must maintain the EIS credential per ECI-HHSC requirements yearly to remain an employee with the agency 6. Must review the ECI program internal task requirements for all required activities as an EIS 7. Other duties as assigned

    MINIMUM REQUIREMENTS
    1. Must be registered with ECI-HHSC State office as an EIS fully qualified professional in the State database with an active credential on file and no ethical violations recorded with the State office credentialing database. 2. Proficiency with standard Microsoft Office applications and system databases 3. No less than six (6) months full-time experience in an Early Childhood Intervention Program 4. Must maintain required credentials and mandatory training requirements to ensure compliance with all State regulations and CHCS policies 5. Must maintain a valid driver’s license and automobile insurance coverage, be able to travel as needed, and be able to meet on a consistent basis the driving record requirements of the Company’s auto insurance carrier if you drive your vehicle during company business 6. Must be able to meet the physical requirements to First Aid and Infant CPR training including lifting up to 12 lbs. and supporting up to 55 lbs.; bending, stooping and getting on and off the floor without assistance 7. Must have adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), using fingers, hands, feet, legs and torso in various care 8. The employee must be able to regularly lift and/or move up to 40 pounds and occasionally must lift and/or move up to 50 pounds 9. The employee must be able to occasionally transfer a baby or toddler from the family home floor to the couch; stroller or high chair and/or from the floor at the daycare to an infant or toddler seat or crib in the classroom
