    About the Service Coordinator - Case Manager Role
    As a Service Coordinator within our Early Intervention team, you'll play a vital role in supporting children under the age of 3 who have developmental delays by coordinating access to needed services and supports. This position is ideal for someone who is compassionate, organized, and culturally responsive, and who thrives in fast-paced environments where collaboration matters.

    Starting Rate: $20.00/hour, based on experience and qualifications

    Service Coordinator - Case Manager Core Responsibilities

    • Request and complete service authorizations in a timely manner
    • Conduct Family Service Plan meetings and ensure compliance with OCDEL standards
    • Coordinate services and make referrals to support each child’s development
    • Participate in Multi-Disciplinary Team meetings to promote collaborative planning
    • Develop and update transition plans to maintain continuity of care
    • Maintain positive and professional relationships with families, providers, and community partners
    • Document all services and communication accurately and on time

    What You Bring

    • Bachelor’s degree in Early Childhood Education, Psychology, or a related field and at least 1 year of experience in a human services role;OR an associate’s degree or 60 college credits in Early Childhood Education and 3 years of relevant experience
    • Strong customer service and time management skills
    • Ability to navigate digital systems and prioritize multiple tasks
    • A positive, solution-focused attitude and ability to build rapport
    • A valid driver’s license and reliable transportation

    Location & Schedule
    This position is based in Luzerne and Wyoming counties, Pennsylvania. Work is primarily community-based with some remote flexibility.
    Office hours are Monday through Friday, 8:30 AM to 4:30 PM. Occasional evening work may be required based on family needs.
