Job Role Summary
Responsible for providing a continuum of social work services in an assigned work setting that will assist patients/clients and their families find practical ways of resolving social, emotional, and environmental problems that interfere with optimum functioning and attainment of total health. Works closely with other community agencies and service providers to maximize health outcomes for individuals, families, and communities. This position works primarily in the District Health Offices; and the majority of services provided are home or community based.

Communication
Strong oral and written communication skills.
Navigation of Computer Systems
Strong ability to use a variety of computer systems such as Microsoft 365, electronic health records, etc.

Ability to work in several settings and demonstrate flexibility to meet the needs of the community.
Working Environment
Bloodborne Pathogens: exposure to blood or other potentially infectious material
Extreme Weather: exposure to extreme temperatures or outdoor elements.
Respiratory protection: exposure to inhalation hazards requiring the use of a respirator.
Travel between offices and throughout the community.
