Location: On-site at University Hospital
Position: Disease-Focused Oncology Nurse Navigator
Shift: 1.0 FTE Day Shift (4 - 10 hour shifts) 6:00a-4:00p
The Disease-Focused Oncology Nurse Navigator coordinates the care needs of assigned patients and develops comprehensive plans to manage care delivery across the patient care continuum. The role involves partnering with patients and their primary physicians to create customized care plans based on individual needs and preferences. The Navigator collaborates with physicians, nurses, health professionals, social workers, and other team members to ensure appropriate tests and treatments are delivered in a timely manner. The role also advocates for the patient and balances care needs with financial considerations to ensure efficient and effective treatments.
Position Overview:
The Oncology Nurse Navigator manages patient care coordination by developing personalized care plans, collaborating with multidisciplinary teams, and ensuring timely delivery of services. This position acts as a patient advocate, supports treatment adherence, and promotes seamless communication among care providers to optimize patient outcomes. The Navigator plays a vital role in facilitating care continuity, educating patients and families, and ensuring compliance with healthcare policies and regulations.
