The Behavioral Health Discharge Planner provides discharge planning, financial counseling, psychosocial assessments, and limited emotional support for patients/families as indicated. Remains knowledgeable of community resources and refers patients appropriately to community agencies and post-acute providers. The discharge planner provides these services to adult and geriatric patients in an inpatient setting Duties Responsibilities Creates a process for communicating and collaborating with members of the inter-disciplinary team involved in the care of patients. Participates with the unit team in daily rounds with the patients and reviews assessments, sets goals, and prioritizes assignments. Screens patients admitted and assesses those patients at high risk for post discharge needs. Serves as a liaison for patients/families, nursing staff, and physicians. They are the vital link for community, health, and social agencies that have previously provided services for the patient or will be providing services post discharge. Provides the Medicare Important Message to patients prior to discharge as appropriate. Educates the patient and family on the discharge planning process and facilitates patient flow through the healthcare continuum. Arranging services in other medical facilities, other specialized institutions, or community agencies. Assists the patient and/or family in applying for financial assistance, services, or counseling to the appropriate community agency or other source of help. Validates payer coverage and assures appropriate agencies are utilized. Maintains expertise of available resources and federal and state medical assistance programs. Screens for abuse, neglect, or exploitation and refer appropriately to state or community agencies in accordance with state and federal regulations. Collaborate with team members to identify and validate avoidable days. Maintains professional integrity and confidentiality of information in accordance with AnMed policies and procedures, state and federal requirements, and applicable laws. Adapts quickly to changes in criteria and/or focus and fulfills duties responsibly with little supervision. Able to work with variable patient populations. Stays abreast of departmental and organizational changes. Recommends process improvements of policies and procedures as necessary. Performs other duties as assigned. Qualifications Bachelors degree in social sciences or a four-year degree with related experience. Knowledgeable regarding community resources. Excellent communication skills. Works in collaborative team approach. Preferred Qualifications Two years of hospital discharge planning. One to two years of experience in social services or patient care experience.
