A community health organization in New York City is seeking an experienced professional to join their team as the new Director, Support Services. In this role, the Director, Support Services will lead support services focused on case management, HIV care coordination, and food insecurity programming. This role oversees team operations, client intake, service planning, and quality-focused program delivery across Staten Island with occasional Queens visits.

- Hours: 9:00 AM to 5:00 PM

Responsibilities:
- Oversee client intake, eligibility verification, and assignment of new members into the program.
- Conduct initial assessments, reassessments, and service plan reviews to determine client needs.
- Direct and support staff through one-on-one supervision, coaching, feedback, and performance development.
- Coordinate case reviews, interdisciplinary meetings, program recruitment, and ongoing client navigation needs.
- Monitor documentation, quality assurance, performance data, and program reporting to ensure compliance and progress toward goals.
Qualifications:
- At least 4 years of direct service experience in HIV care management.
- At least 2 years of supervisory experience managing 2 or more employees, or equivalent additional direct service experience.
- Bachelor's degree, or additional qualifying experience in lieu of a degree.
- Experience with case management and care management practices.
- Knowledge of care management documentation, referral processes, and follow-up.
- Strong oral and written communication skills.
- Ability to organize priorities, manage multiple tasks, and maintain accuracy and attention to detail.
- Ability to assess staff work, provide constructive feedback, and delegate tasks effectively.
- Experience with Microsoft Office applications, including Outlook, Teams, Word, and Excel.
Desired Skills:
- Experience in health homes or social services programs.
- Familiarity with EHR systems such as Epic or Salesforce.
- Experience with HIV education, treatment adherence, and related client support.
- Background in writing workflows or policies and procedures.
- Bilingual English/Spanish skills.
