We are seeking an experienced and compassionate Director of Social Services to lead our social services program and serve as a trusted advocate for residents and their families.

    In this leadership role, you will oversee the social services function, coordinate care transitions, support resident well-being, and collaborate with interdisciplinary teams to ensure exceptional, person-centered care.     Key Responsibilities

    Leadership & Resident Advocacy

    • Lead and oversee Social Services operations, ensuring high-quality, resident-centered care and compliance with all applicable regulations.
    • Advocate for residents' psychosocial needs and support residents and families throughout their healthcare journey.
    • Build strong relationships with residents, families, community organizations, and healthcare partners.

    Care Planning & Discharge Coordination

    • Direct and participate in resident assessments, care planning, and discharge planning activities.
    • Promote safe and successful transitions through ongoing communication with residents, families, and care teams.
    • Develop and implement individualized social service plans that support resident goals and quality of life.

    Program Development & Community Engagement

    • Collaborate with residents, families, and community partners to develop programs that enhance resident engagement and well-being.
    • Serve as a resource to staff and community agencies in addressing resident concerns and social service needs.
    • Assist residents and families in accessing community resources and financial assistance programs, including Medicare, Medicaid, and other available benefits.

    Administrative Responsibilities

    • Complete and oversee required documentation, assessments, reports, care plans, and related regulatory requirements.
    • Coordinate transportation, referrals, and services with internal departments and external providers.
    • Ensure social service practices align with organizational objectives and industry best practices.

    Minimum Qualifications

    • Bachelor's or Master's degree in Social Work, or a Bachelor's or Master's degree in a human services-related field, including Sociology, Psychology, Rehabilitation Counseling, or Special Education.
    • Minimum one year of supervised social work experience in a healthcare setting working directly with individuals.
    • Experience in long-term care, senior services, and dementia support.
    • Current Michigan Social Work licensure required.
    • Demonstrated leadership ability with exceptional customer service and interpersonal skills.
    • Strong communication, organizational, and problem-solving abilities.
    • Ability to thrive in a fast-paced, resident-focused environment while adapting to changing priorities.
