Wyndemere''s 5-star skilled nursing facility is seeking a Social Services Director for a smaII, 56 bed life- plan senior community. This role coordinates and supports all social services related functions within the Social Services Department in accordance with current federal, state, and local standards. Assesses, plans, implements, and evaluates the social, emotional, and psychological needs of sub-acute and long-term residents. Coordinates all Health Center admissions and discharges through review with patients and families.
The Social Services Director leads the psychosocial services program for residents in our Skilled Nursing Facility. This role partners with residents, families, physicians, and the interdisciplinary care team to provide advocacy, care coordination, discharge planning, and emotional support throughout the resident journey.
The Social Services Director plays a key role in helping residents achieve their highest level of well-being while facilitating smooth transitions across the continuum of care, including Skilled Nursing, Assisted Living, Memory Care, Independent Living, hospitals, and community-based services.
Key Responsibilities
- Provide psychosocial support, counseling, advocacy, and resource coordination for residents and families.
- Assess, monitor, and address residents'' emotional, behavioral, social, cultural, and spiritual needs.
- Lead discharge planning and care transitions, ensuring residents and families are prepared for successful outcomes after discharge.
- Collaborate with nursing, physicians, rehabilitation, admissions, case management, and community providers to coordinate resident care.
- Educate residents and families regarding Medicare, Medicaid, insurance coverage, hospice services, and community resources.
- Coordinate referrals to home health, behavioral health, hospice, and other support services.
- Participate in care conferences, resident risk meetings, and interdisciplinary team meetings.
- Maintain compliance with CMS, state, and regulatory requirements, including MDS assessments, psychosocial documentation, and care planning.
- Advocate for resident rights and assist with grievance resolution.
- Support quality improvement initiatives, hospital partnerships, and programs focused on improving resident outcomes and reducing readmissions.
- Provide support to residents and families during significant life events, including cognitive decline, hospice transition, end-of-life planning, and bereavement.
- Maintain confidentiality and comply with HIPAA and professional ethics standards.
- Promote a culture of exceptional customer service, teamwork, and resident-centered care.
Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Gerontology, or a related field required.
- Minimum of 3 years of experience in long-term care, skilled nursing, or geriatric services; 5+ years preferred.
- Strong knowledge of Medicare, Medicaid, CMS regulations, resident rights, and discharge planning processes.
- Experience working with older adults, including those with dementia, behavioral health needs, and complex psychosocial concerns.
- Excellent communication, relationship-building, conflict resolution, and organizational skills.
- Ability to collaborate effectively with residents, families, healthcare providers, and community partners.
