    The Director of Patient Care Services provides overall clinical leadership and operational oversight for home health services. This role is accountable for ensuring the delivery of safe, high-quality, compliant patient care while supporting clinicians through effective supervision, coaching, and performance management.     Overview
    We re seeking an experienced Registered Nurse (RN) to lead our home health clinical team as Director of Patient Care Services. In this role, you ll drive high-quality patient care, ensure regulatory compliance, and support a collaborative, patient-centered environment.

    Responsibilities
    • Lead and supervise all home health clinical operations and staff
    • Ensure quality patient care in accordance with physician orders and care plans
    • Maintain compliance with Medicare, Medi-Cal, and state regulations
    • Oversee staff hiring, training, performance, and scheduling
    • Drive performance improvement and ensure accurate clinical documentation
    • Coordinate interdisciplinary care and serve as a key liaison with physicians
    • Manage patient admissions, discharges, and overall care delivery


    Minimum Qualifications:

    - Registered Nurse with current licensure in the practicing state
    - Current CPR certification
    - Minimum of two years of home health experience
    - Minimum of two years of clinical leadership, management, or supervisory experience preferred
