    Position Summary

    The Director of Nursing (DON) is responsible for the overall clinical leadership, supervision, and coordination of patient care services within the home health agency. The DON ensures compliance with all federal, state (Nevada), and accreditation standards, while promoting high-quality, patient-centered care.

    This role works closely with the Administrator and oversees all clinical staff, ensuring safe, effective, and efficient delivery of home health services.

    Key Responsibilities

    Clinical Oversight

    • Direct and supervise all clinical operations and patient care services
    • Ensure development, implementation, and review of individualized care plans
    • Monitor patient outcomes and ensure quality improvement initiatives
    • Review and approve clinical documentation for accuracy and compliance

    Staff Supervision & Leadership

    • Supervise and support RNs, LPNs, CNAs, and other clinical staff
    • Conduct hiring, training, evaluations, and disciplinary actions
    • Ensure staff competency, ongoing education, and compliance with licensing requirements
    • Provide clinical guidance and mentorship

    Regulatory Compliance

    • Ensure compliance with Medicare Conditions of Participation (CoPs), Nevada state regulations, and accrediting bodies (CHAP, ACHC, Joint Commission)
    • Maintain readiness for audits, surveys, and inspections
    • Oversee infection control and safety programs

    Quality Assurance & Performance Improvement (QAPI)

    • Develop and implement QAPI programs
    • Track key performance indicators (KPIs) and clinical outcomes
    • Identify areas for improvement and implement corrective actions

    Operations & Coordination

    • Collaborate with physicians, case managers, and interdisciplinary teams
    • Oversee patient admissions, recertifications, and discharges
    • Ensure appropriate staffing levels and case assignments
    • Assist in policy and procedure development

    Documentation & Reporting

    • Ensure timely and accurate OASIS documentation and clinical records
    • Maintain compliance with electronic health record (EHR) systems
    • Report clinical issues, incidents, and outcomes to leadership

    Qualifications

    • Active Registered Nurse (RN) license in Nevada
    • Minimum 2–3 years of home health experience (required)
    • At least 1 year in a supervisory or leadership role
    • Strong knowledge of Medicare, OASIS, and home health regulations
    • Excellent leadership, organizational, and communication skills

    Preferred Qualifications

    • Bachelor of Science in Nursing (BSN) or higher
    • Experience with CHAP accreditation or Medicare surveys
    • Experience with home health software (e.g., Axxess, WellSky, Kinnser)

    Reporting Structure

    • Reports directly to the Administrator / Owner
    • Oversees all clinical staff and patient care operations

    Work Environment

    • Office-based with field visits as needed
    • Fast-paced healthcare environment requiring compliance and accuracy
