Ongoing Patient Oversight

The DON is the primary clinical nursing leader responsible for the patient census. For each patient, this includes the initial assessment, Plan of Care development and updates, 30-day check-ins, 60-day reassessments, review of changes in condition, clinical direction to assigned staff, documentation and RN sign-off, and break-in-care assessments when required. As the patient census grows, with approximately two to four new patients anticipated per month, these clinical responsibilities continue for all active patients.

Emergency Clinical Availability

The DON will remain available by phone for urgent clinical matters on an emergency, as-needed basis during operating hours of 8:00 AM to 8:00 PM. On-call responsibility is not 24/7 and does not require overnight availability. During operating hours, the DON will respond to clinical questions involving active patients, provide immediate nursing direction when needed, determine when an in-person assessment is necessary, coordinate appropriate clinical follow-up, and document emergency clinical involvement.

Work Location and Travel

This is a traveling, field-based position serving patients in their homes throughout Central and South New Jersey. In-person visits include initial assessments, start-of-service visits, 60-day reassessments, break-in-care assessments, and emergency clinical visits when required. The DON will also report to the Kearny, New Jersey office approximately once per month for meetings, clinical review, training, or documentation review. These functions are handled by First Call Wellness leadership and administrative, intake, and billing personnel.

Clinical Documentation and NPI

The DON will complete the clinical documentation required for patients under their professional responsibility.
