About the Position


Caretruly Healthcare is seeking an experienced and dedicated Director of Nursing Services (DON) to lead our Nursing Department and oversee clinical services across our Personal Care Services (PCS) and Skilled Home Health programs.


The DON will provide clinical leadership, supervise nursing operations, support staff development, monitor quality of care, and ensure services are delivered safely and in compliance with applicable regulations, professional standards, payer requirements, and agency policies.

Responsibilities


  • Provide leadership, direction, and oversight to the Nursing Department.

  • Supervise and support RNs, LPNs/LVNs, and other clinical personnel.

  • Oversee nursing assessments, reassessments, supervisory visits, and care planning.

  • Ensure client care plans and services are consistent with assessed needs, authorized services, and applicable requirements.

  • Oversee clinical services provided through PCS and skilled home health.

  • Monitor high-risk clients, changes in condition, incidents, and clinical concerns.

  • Ensure appropriate follow-up and communication with physicians, care managers, families, and other healthcare professionals.

  • Monitor nursing documentation for accuracy, completeness, and compliance.

  • Ensure nursing staff maintain required licenses, certifications, training, and competencies.

  • Participate in recruitment, onboarding, performance evaluations, and professional development of nursing staff.

  • Support quality assurance and performance improvement initiatives.

  • Review clinical incidents, complaints, and quality concerns and assist with corrective action when needed.

  • Oversee infection prevention and client safety practices.

  • Support regulatory compliance, audits, surveys, and required corrective actions.

  • Promote client rights, dignity, safety, and quality of care.

  • Collaborate with agency leadership and other departments to improve clinical operations and client outcomes.

Qualifications


  • Current, unrestricted Registered Nurse (RN) license in the state of Wisconsin.

  • Significant clinical nursing experience.

  • Previous nursing leadership, supervisory, or management experience.

  • Strong knowledge of nursing standards, assessments, care planning, and clinical documentation.

  • Excellent leadership, communication, organization, and problem-solving skills.

  • Ability to manage multiple priorities and work effectively in a fast-paced environment.

  • Strong clinical judgment and decision-making skills.

Preferred Qualifications


  • BSN or higher.

  • Experience in home health, personal care, or other community-based healthcare settings.

  • Experience with Medicaid/MCO and/or Medicare programs.

  • Experience with quality assurance, regulatory compliance, or clinical audits.

  • Experience with electronic medical records.
