    Job Summary:As the Director of Hospital Case Management, you will oversee and coordinate all departmentalactivities to ensure compliance with organizational policies and regulatory agencies. Your leadershipwill drive the integration of patient care and discharge planning processes, optimizing clinical resourceutilization and ensuring continuity of care. You will manage daily operations, engage interdisciplinaryteams, and achieve established outcomes, all while maintaining an executive presence and excellentcommunication skills.
    Location: Denver, COCompensation: Mid-Range: $146,000 + 15% Potential Bonus IncentiveSign-On Bonus: $10,000Relocation: Case by Case BasisSchedule: Full-Time, No Weekends
    Qualifications:
    • Bachelor’s degree in Nursing (required)
    • Master’s degree in Nursing, Health Administration, or Business Administration (preferred)
    • 3+ years of overall acute care hospital case management experience
    • 2+ years in case management leadership in an acute care setting
    • Previous HCA experience highly preferred
    • Extensive leadership experience in case management is essential
    Team Structure:
    • 2 Managers
    • 30 Case Management Staff
    Key Responsibilities:
    • Direct and evaluate departmental operations, including staffing, information technologies,
    • onboarding, and staff competencies.
    • Allocate resources to meet productivity and quality goals.
    • Oversee case management functions and ensure adherence to care coordination and discharge
    • planning processes.
    • Collaborate with the Facility CFO and Division CM Leader to achieve goals.
    • Assess and improve department performance through data analysis and performance
    • improvement activities.
    • Ensure compliance with policies and standard operating procedures.
    • Establish working relationships with key stakeholders including CMO, CNO, and ancillary service
    • leaders.
    • Perform other duties as assigned and adhere to the "Code of Conduct" and "Mission and Value
