    SUMMARY Under the direction of the Chief Operating Officer, the Director of Community Supports will play a crucial role in leading and overseeing the Housing Navigator teams, implementing strategic initiatives, and fostering community engagement across multiple markets. The ideal candidate will have a strong background in the medical field, coupled with extensive experience in community resource outreach.

    WHERE YOU'LL WORK Some travel is required to meet with staff and members within designated areas or based on business needs. You are eligible for mileage reimbursement for the use of your vehicle for business-related travel. Standard business hours are Monday through Friday from 8:00 am to 5:00 pm.

    WHAT YOU'LL DO Team Leadership

    Provide visionary leadership to the Community Supports teams, ensuring alignment with company goals and objectives

    Foster a collaborative and high-performance culture within the Community Supports department

    Develop and implement training programs to enhance the skills and knowledge of Community Supports and Housing Navigator team members

    Provide mentorship and training to direct reports to promote growth within the department

    Strategic Planning

    Collaborate with executive leadership to develop and execute Community Supports strategies that align with Titanium Healthcares mission and vision

    Develop and implement strategies to effectively engage diverse communities and markets

    Analyze market trends and competitor activities to identify opportunities and challenges

    Provide administrative oversight of monthly contract billing and tracking of outcomes ensuring that KPI and weekly reports have accurate data and are presented in a timely manner

    Responsible to manage, track, and improve all Community Supports quality initiatives

    Community Engagement

    Cultivate and maintain relationships with key stakeholders in the healthcare community, including medical professionals, community organizations, and government agencies

    Represent Titanium Healthcare at community events and establish the company as a trusted partner in healthcare services

    Outreach Activities

    Oversee the planning and execution of Community Supports activities, ensuring they are aligned with the companys objectives and compliant with relevant regulations

    Monitor and analyze performance metrics, providing regular reports to executive leadership

    Cross-Functional Collaboration

    Collaborate with other departments, including marketing, sales, and operations, to ensure a cohesive and integrated approach to all Community Supports initiatives

    Work closely with internal teams to identify opportunities for improvement and innovation in Community Supports strategies

    Perform other duties and projects as assigned

    WHO YOU ARE

    Strong understanding of the CalAIM program and Community Supports resources Experience working with community resources and building strategic partnerships Detail oriented with excellent time management, business acumen and data driven

    Excellent interpersonal and communication skills Ability to develop and execute strategic plans Supervisory or managerial experience leading a team of direct reports

    Sensory Requirements

    Fluent in English (written and verbal) Ability to communicate clearly in-person, by phone, and electronically Adequate hearing and vision (with corrective devices if necessary) to conduct assessments and documentation Ability to identify problems and use logic and related information to develop and implement solutions Excellent organizational skills and attention to detail Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare practice

    Physical Activity

    Must be able to remain in a stationary position Must be able to move around the office and/or travel throughout community Ability to operate a vehicle and travel to meet with members around the community; attend meetings and events as required or requested

    Environmental Conditions

    Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures

    Technology Use

    Frequent use of computers, keyboard, and handheld/mobile devices Ability to type for extended periods Competent with computers, email, virtual platforms, and Microsoft Office based programs

    WHAT YOU'LL NEED

    Bachelor's degree in business, health administration, public health, social services or another related field Minimum 5+ years progressive experience in business development and 3+ years of experience in healthcare administration

    Master's degree Bilingual in Spanish
