Position Summary

The Director of Clinical CaseManagement is responsible for the planning, organization, direction, supervision and evaluation of all Clinical Care Management and Chaplaincy services provided at Craig Hospital. This position partners with leaders of the hospital to develop and align CCM (Clinical Care Management) and Chaplaincy strategies to address business needs that maximize the effectiveness of CCM programs. This position will assess and anticipate CCM and Chaplaincy-related needs, effectively partner to develop integrated solutions and clinical strategies to meet the goals of the hospital.

Position Overview & Leadership Focus

  • Strategic Leadership: Direct and mentor a dedicated team of Certified Case Managers (CCMs) who partner with interdisciplinary medical teams to comprehensively coordinate care.
  • Specialized Patient Population: Oversee case management operations specializing in life-changing rehabilitation for individuals recovering from Spinal Cord Injuries (SCI) and Brain Injuries (BI).
  • The Mission: Champion the systems, compliance, and clinical pathways that support our team in assisting patient healing and achieving the ultimate goal of getting patients back to life.

Standard Business Hours:Full-time; Monday-Friday, 8 am-4:30 pm MST

Compensation:$108,076.80—$155,076.80; Exempt

Position Requirements:

  • Education:
    • Master of Science from an accredited College or University in Clinical Counseling or related field required.
  • Experience:
    • 5-7 years recent experience as a social worker or counselor required.
    • 1-3 years prior leadership experience required.
    • Experience in healthcare strongly preferred
  • Skills & Abilities:Excellent organization and customer service skills. Ability to successfully handle multiple priorities and prioritize and schedule accordingly. Must be able to successfully and effectively communicate with a variety of personnel. Advanced supervisory, team-building, conflict resolution, and communication skills, both verbal and written are required. Ability to determine or interpret work procedures for a group of workers, assigning specific duties to them, maintaining harmonious relations among them and promoting efficiency. Work collaboratively with other departments and physicians. Able to respond to crisis' and emergency situations appropriately. Ability to develop institutional educational and research policies and programs, quality improvement programs, and operational policies and procedures. Ability to use professional judgment to identify areas for improvement and participate in development of quality initiatives. Ability to maintain confidentiality in all aspects of the position. Proven analytical, problem-solving, and troubleshooting skills. Ability to make formal presentations in front of large groups. Creative and able to be an "out of the box" thinker. Strong computer skills utilizingtheMicrosoft Suite.

Essential Functions:

  • Provide direction and supervision of department personnel, demonstrate professional judgment, use problem-solving techniques and delegate appropriately. Provide direction for new program development, new treatment techniques and equipment.
  • Provide leadership in the development of performance standards for all departmental personnel.
    Evaluate and counsel all employees on their performance in a professional and timely manner. Plan and develop educational in-services for the department as appropriate.
  • Responsible for the implementation and coordination of department procedures based on the policies and procedures of the department. Review and analyze reports, records, and directives and confer with coworkers to obtain data required for planning department activities.
  • Prepare annual budget and monitor concurrently. Monitor and analyze costs associated with supplies, staffing, etc.
  • Provide and organize individual and group staff educational opportunities, maintaining department's competency level. Develop and manage proficiency of annual staff competency expectations.
  • Negotiate service contracts with staffing agencies, schools, etc.
  • Provide staff education/precepts new staff hires, new staff, interns and students.
  • Provide a staffing matrix that effectively provides adequate coverage. Collaborate with the team to assess and allocate staff resources on a daily basis, based on patient census, acuity and patient need. Analyze employee workload and adjusts as necessary.
  • Delegate and/or perform the essential job duties and responsibilities of the department staff as relates to direct patient care, including treatment of patients, adapting the treatment plan as needed.
  • Plan, organize, direct, coordinate and oversee the daily operations of the in and outpatient CCM departments and the Chaplaincy programs.
  • Develop and maintain a good working rapport with intra-department personnel and meet with administration, medical and nursing staff, and other related departments for collaboration and planning. Coordinate department activities, planning of patient care services and patient care plan with the Medical Staff and other departments of the Hospital.
  • Work with patients and families to provide support and information for taking a more proactive role in self advocacy to improve the quality of life/care for individual patients.
  • Ensure that CCM staff work with patients, families, and the interdisciplinary team to anticipate and plan for medical and resource needs at discharge.
  • Participate in the development and implementation of department goals and objectives. Sen e as a change facilitator, assisting others to understand the importance, necessity and impact of change.
  • Consistently demonstrate knowledge of and application of quality indicators, and proactively monitor and implement systems to achieve and/or surpass hospital benchmarks. Coordinate and/or develop ongoing QA activities for department to monitor.
  • Support and promote Hospital standards for superior customer service by exhibiting positive, courteous, and helpful behavior when dealing with all internal and external customers.
