A leading healthcare organization is seeking an experienced Director of Case Management to oversee and strengthen hospital-based case management operations. This leader will play a key role in improving patient experience, discharge planning effectiveness, utilization management, and building a high-performing Case Management team. Job Details

Lead all Case Management department operations within the hospital
Oversee utilization review, discharge planning, and care coordination activities
Manage and mentor a team of Case Managers
Recruit, onboard, and develop new Case Management staff
Support improvements in patient experience and discharge planning outcomes
Collaborate with physicians, nursing leaders, therapists, and interdisciplinary teams
Monitor departmental performance metrics and implement process improvements
Ensure compliance with regulatory and accreditation requirements
Assist with utilization management initiatives and resource optimization
Carry a patient caseload as needed while supporting department operations
Foster a positive culture and high-performing Case Management environment
Partner with executive leadership to achieve organizational goals
