Job Details / Responsibilities:
Lead daily operations and human-resource management of the Case Management department
Direct utilization review, care coordination, and discharge planning across a 40-bed all-private inpatient rehab hospital
Supervise 2 full-time Case Managers and 2 contract Case Managers; recruit and onboard 2 additional full-time Case Managers
Build a new CM team and drive a positive culture change focused on the discharge-planning experience
Improve patient-experience and discharge scores through process, coaching, and accountability
Ensure assessments are completed independently within each clinician’s scope of practice (RN, SW, OT, PT, ST, Rehabilitation Counseling)
Partner with hospital leadership, therapy, nursing, and physicians on throughput, payer requirements, and safe transitions
Maintain compliance with hospital, state, and payer standards for UR and discharge planning
Provide or oversee first-year preceptor support for the department
Qualifications:
Qualified to independently complete an assessment within the scope of practice of the discipline (RN, SW, OT, PT, ST, or Rehabilitation Counseling)
Active license in the hospital’s state if licensure exists for the discipline
RN path (strongly preferred): BSN and active RN license
Non-RN path: bachelor’s degree required; graduate degree preferred; social workers must hold an active social work license and have medical/hospital experience
3 years of hospital-based Case Management experience, including Utilization Review and Discharge Planning
CCM or ACM required, or obtained within the first year
Candidates coming from large health-system models must demonstrate solid discharge-planning experience (IRF-style discharge planning is the priority)
