Overview
As the Director of Care Management, you will play a pivotal role in ensuring the delivery of high-quality patient care and effective care coordination within our primary care setting. You will lead a team of care managers and other care team support members and collaborate closely with healthcare providers to optimize patient outcomes and enhance the overall patient experience.

Qualifications
Bachelor''s degree in nursing required, Master''s degree in a related health field preferred
Proven experience in care management, care coordination, or population health management within a primary care setting.
Current licensure as a Registered Nurse (RN) required, compact state licensure preferred
Demonstrated leadership abilities, with a track record of effectively leading and developing high-performing teams and delivering results for all.
Strong interpersonal, communication, and collaboration skills, with the ability to build rapport and relationships with diverse stakeholders.
Excellent analytical and problem-solving skills, with the ability to interpret data and implement data-driven solutions.
Knowledge of healthcare regulations, quality standards, and best practices related to care management and population health management.
Ability to thrive in a fast-paced, dynamic environment and adapt to changing priorities and challenges.
Commitment to continuous learning and professional development in the field of healthcare management and patient care.
