Healthcare United is seeking a Lead Primary Care Physician to join an established healthcare organization in Dayton, Ohio. This is a full -time outpatient primary care opportunity with a Monday through Friday schedule, 8:00 AM to 5:00 PM. <\/p>
This role is ideal for a Primary Care Physician who enjoys direct patient care, clinical leadership, and working within a team -based care model. The Lead PCP will provide care for an assigned patient panel while helping support clinical staff, providers, and interdisciplinary team members. <\/p>
Job Description<\/b> <\/p>
Full -time Lead Primary Care Physician opening in Dayton, Ohio <\/li>
Monday through Friday schedule <\/li>
8:00 AM to 5:00 PM <\/li>
Outpatient primary care setting <\/li>
Provide primary care for an assigned patient panel <\/li>
Manage acute and chronic medical conditions <\/li>
Support preventive care, wellness visits, and follow -up care <\/li>
Work closely with nursing, APPs, care management, pharmacy, behavioral health, and support staff <\/li>
Help lead and support the clinical team <\/li>
Mentor and collaborate with physicians and Advanced Practice Providers <\/li>
Participate in care planning and population health initiatives <\/li>
Support quality metrics, patient outcomes, and continuity of care <\/li>
Provide same -day, next -day, and urgent visits as needed <\/li>
Participate in rotating call coverage as required <\/li><\/ul>

Requirements<\/h3>
MD or DO <\/li>
Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Primary Care -related specialty <\/li>
Active Ohio medical license or ability to obtain <\/li>
Primary care experience required <\/li>
Leadership experience preferred <\/li>
Experience with complex patient populations preferred <\/li>
Strong communication and team -based care skills <\/li>
Interest in value -based care and population health <\/li>
