A nonprofit services agency is seeking a Director for its Client Services team in Staten Island, NY. This role leads care coordination operations, oversees staff, and supports client-centered program delivery in a healthcare setting.

About the Opportunity:

  • Shift: Day shift

  • Schedule: Monday through Friday

  • Hours: 9:00 AM to 5:00 PM

  • Setting: In-person with 1 remote day per week

Responsibilities:

  • Oversee patient navigation and care management activities for the program.

  • Supervise and support navigators through one-on-one coaching, feedback, and development.

  • Lead enrollment, assessment, care planning, and reassessment processes.

  • Collaborate with medical providers and care coordination staff to support recruitment, enrollment, and ongoing client needs.

  • Monitor program outcomes, documentation quality, and reporting to support compliance and continuous improvement.

Qualifications:

  • At least 4 years of direct service experience in HIV care management.

  • At least 2 years of experience supervising two or more employees, or 10+ years of direct services experience to the target population in lieu of supervisory experience.

  • Bachelor's degree required, though additional relevant experience may be considered in lieu of a degree.

  • Strong knowledge of care management principles, documentation, referral processes, follow-up, and treatment adherence strategies.

  • Experience with Microsoft Outlook, Teams, Word, and Excel.

  • Excellent oral and written communication skills with the ability to manage multiple priorities and maintain accuracy.

Desired Skills:

  • Experience with HIV care coordination or related health homes and social services programs.

  • Experience with AWARDS database.

  • Bilingual English/Spanish skills.

  • Experience with quality assurance, quality improvement, and performance reporting.

  • Experience working directly with healthcare providers and interdisciplinary care teams.
