A nonprofit services agency is seeking a Director for its Client Services team in Staten Island, NY. This role leads care coordination operations, oversees staff, and supports client-centered program delivery in a healthcare setting.
About the Opportunity:
Shift: Day shift
Schedule: Monday through Friday
Hours: 9:00 AM to 5:00 PM
Setting: In-person with 1 remote day per week
Responsibilities:
Oversee patient navigation and care management activities for the program.
Supervise and support navigators through one-on-one coaching, feedback, and development.
Lead enrollment, assessment, care planning, and reassessment processes.
Collaborate with medical providers and care coordination staff to support recruitment, enrollment, and ongoing client needs.
Monitor program outcomes, documentation quality, and reporting to support compliance and continuous improvement.
Qualifications:
At least 4 years of direct service experience in HIV care management.
At least 2 years of experience supervising two or more employees, or 10+ years of direct services experience to the target population in lieu of supervisory experience.
Bachelor's degree required, though additional relevant experience may be considered in lieu of a degree.
Strong knowledge of care management principles, documentation, referral processes, follow-up, and treatment adherence strategies.
Experience with Microsoft Outlook, Teams, Word, and Excel.
Excellent oral and written communication skills with the ability to manage multiple priorities and maintain accuracy.
Desired Skills:
Experience with HIV care coordination or related health homes and social services programs.
Experience with AWARDS database.
Bilingual English/Spanish skills.
Experience with quality assurance, quality improvement, and performance reporting.
Experience working directly with healthcare providers and interdisciplinary care teams.
