Responsibilities:- Provide direct care and personal support to individuals with disabilities or special needs in home and community settings
- Assist clients with activities of daily living (ADLs) including bathing, grooming, dressing, and meal preparation
- Implement individualized support plans and document progress accurately
- Transport and accompany clients to appointments, errands, and community activities
- Administer medications and monitor health conditions per care plan guidelines
- Foster independence, dignity, and self-advocacy in all client interactions
- Communicate effectively with supervisors, family members, and care team members
Requirements:- High school diploma or GED required
- Previous experience in direct support, caregiving, or human services preferred
- Valid driver's license and reliable transportation
- Ability to pass a background check and drug screening
- CPR and First Aid certification (or willingness to obtain)
- Strong interpersonal skills with a patient, empathetic, and dependable demeanor
- Ability to work flexible hours including evenings, weekends, or holidays as needed
