STATUS: Full Time;
SCHEDULE / HOURS:  8:00 AM - 4:00 PM, 4:00 PM- 12:00 AM, 12:00 AM - 8:00 AM ALL shifts available!
LOCATION: Multiple sites - Monroeville, Penn Hills, Wilkinsburg areas

 

Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Our Agency oversees 20 residential settings, two outpatient offices, three adult training facilities, a prevocational enterprise program, a consumer drop-in center, habilitation services, and two Health Care Quality Units in Butler and Warren, which train professionals in 18 counties. 

Responsibilities:

This position provides direct care and support to clients with Intellectual Disabilities and Autism (IDA) supported through the Residential Program with their individual goals and activities of daily living, as needed and as defined in their Individual Support Plans (ISPs).

 

  • Demonstrates and reflects a commitment to the mission and values of Milestone by serving others with respect, dignity, and compassion
  • Communicates in writing and orally clearly and effectively with IDA residential clients, peers, Milestone associates and all team members
  • Works with residential clients and their teams to maintain a safe and healthy home environment
  • Expectation to the immediate reporting of any unmet safety or welfare needs by notifying the Community Living Manager, Residential Director, Nurse, or other on-call personnel.
  • Assists the IDA residential clients in the maintenance of the home, including cooking, cleaning, laundry, shopping, and personal hygiene needs.
  • Document goal data, implement goal plans and provide input to individual assessments and goals
  • Provides supervision, support and assistance to IDA residential clients as defined in their ISPs
  • Participate in all required training and maintain required 24-hours of annual training
  • Provides behavioral and crisis support as defined in the Individual Support Plan
  • Maintains an eligible PA, OH, or WV Driver’s license and transports clients as required
  • Actively participates in team and staff meetings
  • Maintains medication administration certification to ensure the ability to safely administer and document medication administration.
  • Assists clients to experience and participate in community activities and outings
  • Completes Daily Service Notes, Communication Notes and Incident Reports as required
  • Reviews and implements all ISPs and related plans (ie. SEEPs, Crisis Plans, Behavior Support Plans, etc.)
  • Utilizes the electronic timekeeping system to record time worked and immediately report instances of missed punches to the Supervisor
  • Reviews and understands licensing regulations to ensure that standards are met
  • Performs work assigned by Community Living Manager
  • Works scheduled shift at assigned site and/or any other site as needed to maintain health and safety of IDA residential clients.
  • Remains on-site to provide support and supervision to residential clients until they are relieved by incoming staff or supervisor
  • Understanding and compliance with all Partners For Quality Corporate Personnel Policies and Procedures and Milestone specific Personnel and Operational Policies and Procedures
  • Other duties as assigned by the Community Living Manager

Qualifications:
  • High School Diploma or GED
  • At least 18 years of age
  • Experience in the field of Human Services, preferred
  • Ability to lift and transfer with or without accommodations
  • Excellent references, both professional and personal
  • CPR, First Aid Certification
  • Medication Administration Certification
  • Act 34 Clearance and FBI Clearance, if applicable
  • Valid Driver’s License with acceptable driving record
Minimum Compensation: USD $17.00/Hr. Maximum Compensation: USD $17.00/Hr.
