Direct Care Worker (DCW): The Direct Care Worker is responsible for providing in-home personal care and daily living tasks as outlined by an Individual Service Plan for individuals who are physically disabled, mentally alert, or mentally challenged. Note: These statements are intended to describe the general nature of the work involved for this job. We have positions open in Wayne and Oakland County.

Pay: $17.13 +(incentive pay available)

Dimensions (Including Physical Demands): The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The employee must constantly talk, see, focus and hear. The employee is frequently required to sit, walk, stand, bend, twist, stoop, kneel, crouch, crawl, push, pull, and reach. The employee must have fine manipulation ability of both hands. The employee must occasionally to frequently lift and/or move 50 to 75 pounds and maneuver a manual wheelchair. The employee is occasionally required to lift and/or move residents of any weight, with assistance of aHoyer lift of other protective equipment and use all equipment effectively. The employee must be able to handle highly stressful situations on a day to day basis. Must have keen sense to detail; Strong interpersonal skills; Strong oral and written communication skills; Requires flexible working hours necessary (Day/Evening/Nights & week-ends); regular, predictable attendance required.

Essential functions and Responsibilities (not an exhaustive list):

  1. Provides personal care services according to assigned schedules and individual service plan content, including but not limited to transferring, bathing, grooming, toileting, preparing meals according to dietary needs, feeding, housekeeping, laundry, errands, and emergency response;
  2. Health maintenance, bowel programs, and wound care may also be performed per state regulations;
  3. Observe and maintain daily notes regarding person's served physical, emotional and social status (recording any symptom, reaction, or response changes);
  4. Dispense medications and properly document information;
  5. Adhere to all health and safety standards to safeguard quality and compliance;
  6. Maintain 100% adherence to all policies and procedures including all state and county requirements including reporting any house/van maintenance issues;
  7. Professional and confidential communication with the Company & the person's served family;
  8. Transport persons served to/from all activities & supervise them as required;
  9. Completion of periodic training programs;
  10. Providing cross training to other employees;
  11. Attending staff meetings and participation in Company events;
  12. Maintain clear and open communication upward, downward, and laterally;
  13. Other duties as required.

Work Environment: The work is done primarily in a home setting however, frequently the employee will travel to various locations. The employee is frequently exposed to wet and/or humid conditions and outside weather conditions including poor driving/road conditions when traveling. The noise level in the work environment is usually moderate.

Other Requirements: Valid Driver's License; Good standing driving record.

Education and Experience: High School Diploma/GED
