10:30 AM – 6:00 PM(7.5-hour shifts)
30 hours per week
Pay: Competitive hourly rate based on experience and qualifications.
Penn Home Health Care is seeking a compassionate, dependable, and professional Direct Care Worker/Caregiver to provide one-on-one, in-home support to an older adult residing at Luther House Senior Independent Living in West Grove, PA.
This is an excellent opportunity for a caregiver looking for a consistent daytime schedule with one participant in a clean, smoke-free apartment.

Assist with bathing, dressing, grooming, and personal hygiene
Provide toileting and incontinence care as needed
Assist with ambulation and mobility using a rolling walker
Provide supervision to help reduce fall risk
Monitor blood pressure using an electronic blood pressure monitor
Prepare light meals and snacks
Perform light housekeeping related to the participant's care
Laundry and linen changes
Provide medication reminders
Assist with approved household tasks as outlined in the care plan
Complete Electronic Visit Verification (EVV) and required documentation
Report changes in condition, safety concerns, or incidents to the agency promptly
Independent senior living apartment
Lives alone
No pets
Non-smoking home
Uses a rolling walker
No Hoyer lift required
Light physical assistance
Experience providing personal care preferred
Direct Care Worker, Caregiver, Personal Care Aide (PCA), Home Health Aide (HHA), or Certified Nursing Assistant (CNA) experience strongly preferred
Compassionate, dependable, and professional
Ability to work independently while following a care plan
Reliable transportation and dependable attendance
Ability to work independently in a participant's home while following an individualized care plan
Must meet all Pennsylvania home care employment requirements
Consistent weekly schedule
One-on-one participant care

Ongoing training and supervision
