POSITION OVERVIEW

The Master Social Worker (MSW) is responsible for providing psychosocial support to patients undergoing dialysis treatment and counseling families on lifestyle adjustments and coping strategies.

Key Responsibilities

• Assess patients financial, medical, social, and familial needs.
• Complete psychosocial assessments and recommend changes in treatment based on the patients current needs.
• Provide case work and group work services to patients and their families in dealing with the special problems associated with end stage renal disease.
• Participate in team review of patient progress and completing monthly care plans on patients.
• Attend monthly Quality Assurance Performance Improvement (QAPI) meetings and participate in the final development of patients Plan of Care on new patients.
• Provide pre-transplant counseling for patients and family members and make necessary arrangements with the transplant center.
• Provide insurance information on new patients and transients to the business office.
• Participate in periodic home visits to complete initial assessments.

Qualifications

• Masters degree in social work (MSW) with a specialization in clinical practice.
• At least two (2) years of experience as a social worker, with one (1) year in a dialysis unit or transplantation program preferred.
• Must meet the licensure requirements, as required, in the state of practice.
