Position Overview
The Diabetic Nurse Educator will play a key role in educating patients, families, and healthcare teams on diabetes prevention, management, treatment plans, and lifestyle modifications. The ideal candidate will possess strong clinical knowledge, exceptional communication skills, and a passion for helping patients successfully manage their diabetes and improve their quality of life.
This role serves a diverse patient population including long-term care residents, swing bed patients, acute care patients, dialysis patients, and outpatient populations.
Job Responsibilities
- Provide comprehensive diabetes education to patients and families across multiple care settings
- Develop individualized diabetes management plans based on patient needs and provider recommendations
- Educate patients on blood glucose monitoring, medication administration, insulin management, nutrition, and lifestyle modifications
- Collaborate with physicians, nurses, dietitians, pharmacists, and other healthcare professionals to optimize patient outcomes
- Assess patient understanding and readiness for diabetes self-management
- Monitor patient progress and adjust educational plans as needed
- Provide education regarding diabetes prevention and risk reduction strategies
- Assist patients in understanding treatment goals and improving adherence to care plans
- Maintain accurate patient documentation and educational records
- Stay current with evidence-based diabetes care guidelines and best practices
- Participate in quality improvement initiatives and community outreach programs when applicable
- Serve as a clinical resource for staff regarding diabetes management and education
Education & Certification Requirements
- Graduate of an accredited Registered Nursing (RN) program
- Current Montana Registered Nurse (RN) license or eligibility for licensure
- Minimum of four (4) years of Registered Nurse experience required
- Strong clinical background in patient education, chronic disease management, or diabetes care preferred
- Certified Diabetes Care and Education Specialist (CDCES) certification preferred but not required
- Current BLS certification preferred
- Excellent communication, presentation, and patient counseling skills
- Ability to work independently and collaboratively within an interdisciplinary healthcare team
Preferred Qualifications
- Experience working with diabetic patient populations
- Background in acute care, outpatient services, dialysis, long-term care, or chronic disease management
- Passion for patient advocacy and preventative healthcare
- Strong organizational and time management skills
- Experience developing educational materials and patient care plans
