Description

The Quality of Care Auditor/Trainer supports the safety, quality, and regulatory compliance of Embrace Life Services programs throughout Arizona. This companywide position reviews services provided through HCBS, group homes, day programs/DTA, therapy services, and supporting administrative departments.

The position conducts program audits, reviews documentation, investigates quality-of-care concerns, provides employee training, identifies service gaps, and monitors corrective actions through completion. This position works closely with program directors, executive leadership.

Job Responsibilities

Essential Responsibilities:

• Conduct scheduled and unannounced quality-control audits at group homes and day programs.

• Perform physical program inspections and desk audits using established company checklists and quality-control standards.

• Review member records, Person-Centered Service Plans, behavior plans, medication records, service documentation, consent forms, staffing ratios, employee training records, and other required documentation.

• Verify that behavior plans and member-specific instructions are current, properly approved, available to employees, and correctly implemented.

• Review incident reports, allegations, member-safety concerns, supervision concerns, documentation deficiencies, medication-related concerns, and potential policy violations.

• Conduct objective quality-of-care investigations and classify findings as substantiated, unsubstantiated, or inconclusive based on available evidence.

• Notify leadership promptly of serious health, safety, compliance, licensing, or member-rights concerns.

• Provide and coordinate employee training related to Article 9, abuse and neglect prevention, member rights, incident reporting, documentation, person-centered services, behavior plans, company policies, and quality-of-care expectations.

• Maintain accurate training attendance, completion, comprehension, and certification records.

• Review program activities, menus, supplies, vehicle records, maintenance needs, and other operational areas that may affect member health, safety, or service quality.

• Assist with records and information requests related to DDD or AHCCCS audits, investigations, licensing reviews, and corrective-action requirements.

• Communicate audit results and concerns through the appropriate directors and keep the CEO and Executive Director informed of significant findings, risks, deadlines, and unresolved issues.

• Travel to Embrace Life Services programs throughout Arizona as assigned.

This position focuses on independent auditing, training, investigations, and corrective-action monitoring. Program directors and managers remain responsible for daily staffing, scheduling, employee supervision, and program operations.

Qualifications

Required Qualifications:

• High school diploma or equivalent.

• Experience working with individuals with intellectual or developmental disabilities, behavioral health services, residential programs, HCBS, day programs, quality assurance, compliance, auditing, investigations, or employee training is preferred.

• Valid driver’s license, reliable transportation, and an acceptable driving record.

• Ability to travel to group homes, day programs, HCBS locations, and other company sites throughout Arizona.

• Ability to meet all Arizona DDD, AHCCCS, background, fingerprint-clearance, training, and certification requirements applicable to the position.

• Ability to obtain and maintain required certifications and training within established company deadlines.

• Availability for occasional evenings or weekends when training, investigations, audits, or urgent program visits are required.

Knowledge and Skills:

• Strong auditing, observation, investigation, and fact-finding abilities.

• Strong understanding of confidentiality, member rights, health and safety, person-centered services, and professional boundaries.

• Ability to remain objective and separate verified facts from assumptions or personal opinions.

• Excellent written communication and report-writing skills.

• Ability to identify compliance concerns, service gaps, documentation errors, and potential risks.

• Ability to explain policies and regulatory requirements clearly to employees and leadership.

• Strong presentation, teaching, coaching, and employee-training skills.

• Strong organization, time-management, follow-up, and deadline-tracking abilities.

• Ability to manage multiple audits, investigations, training assignments, and corrective-action plans.

• Basic computer proficiency, including email, online training platforms, spreadsheets, electronic records, and documentation systems.

• Ability to handle confidential and sensitive information professionally.

• Ability to work independently while collaborating with program directors, Human Resources, Agency Compliance, executive leadership, and regulatory representatives.
