In this role, you will help shape and drive our organizational mission, working closely with internal teams to ensure services are responsive, goal-driven, and compliant with 245D and HCBS standards. You will use data and performance insights to evaluate and improve programs, develop strategic plans alongside Designated Coordinators, and ensure we meet both individual and organizational goals. This is a dynamic leadership opportunity for someone ready to make a meaningful impact while helping our programs grow and thrive.Primary Responsibilities Include:Maintain up-to-date knowledge of 245D licensing requirements and ensure organizational compliance with all applicable regulations.Oversee the implementation and ongoing data tracking of person-centered individual support plans, including Coordinated Service and Support Plans (CSSP), CSSP Addendums (CSSPa), Self-Management Assessments (SMA), and Individual Abuse Prevention Plans (IAPP).Ensure timely completion, organization, and maintenance of all required documentation and signatures in compliance with 245D licensing standards.Prepare and submit quarterly and annual progress reports in alignment with person-centered goals and as requested by individuals served and their support teams. Participate in regular team and staff meetings.Demonstrate strong creative problem-solving skills, addressing challenges independently and collaboratively within a team setting.Facilitate effective communication and coordination across care teams, guardians, and internal departments to ensure continuity and quality of care.Monitor and support the health, safety, and protection from abuse of individuals served, ensuring their overall well-being.Provide oversight of staff onboarding, training, and supervision to ensure service excellence and regulatory compliance.Lead regular performance evaluations and support employee development through coaching and feedback.Serve as a point of contact for staff during and outside of standard business hours to respond to questions, provide guidance, and manage emergencies.Actively contribute to interdisciplinary collaboration as part of a dynamic, person-centered service team.Coverage arrangement and emergency direct care coverage required.Qualifications:Required – Must meet Designated Manager qualifications in accordance with the 245D requirements by one of the following:4-year degree in a human service field and a minimum of 1 year direct care experience2-year degree in a human service field and a minimum of 2 years direct care experienceDiploma/certificate from Post-Secondary School in a human service field and a minimum of 3 years direct care experienceMinimum of 4 years direct care experience under the supervision of a Designated ManagerMinimum of 3-years of progressively responsible supervisory and personnel management experience in Human Services.Preferred:Ability to develop, implement and evaluate client programmingAbility to manage people with different learning stylesAbility to keep a healthy, welcoming working culture for all people working for/with Learnability.Ability to problem solve and conduct scheduling assessments for team membersKnowledge of states and federal requirements as they pertain to the 245D regulationsStrong written and oral skillsExperience with budgeting/financial management License/Certification:Driver's License (Required)Less than 3 moving violations within the last 3 years Shift availability:Morning, afternoon, evening and weekend shift availability Work Location:Hybrid (Mondays work remotely) Compensation: $75,000-$90,000 depending on experience in 245d setting. FLSA Status: Non-ExemptJob Type: Full-timeLocation(s): Twin CitiesSupervisor: Chief Services OfficerSupervises: Designated Coordinators, DSPs/ESPsAcceptable criminal record
