Day Support Program Manager

The PROGRAM MANAGER will supervise direct care personnel and assist with the oversight of staff scheduling to ensure adequate staff coverage to provide individual supports for the program and participate in hiring all CENTERED BASED DAY SUPPORT personnel. The PROGRAM MANAGER shall also provide oversight and ensure the Implementation and documentation of Individual Person-Centered Plans, PCP Support Checklist, and any behavioral Person Centered Plans and programming.

The PROGRAM MANAGER is responsible for the overall day-to-day needs of each 
individual receiving service from IT’S MY LIFE, LLC CENTERED BASED DAY SUPPORT. Other responsibilities and duties include oversight of Individual Person-Centered Plans, PCP checklist and any behavioral Person Centered Plans. The PROGRAM MANAGER will also be responsible for the oversight of the admission, screening / assessment process and oversight of the CENTERED BASED & NON CENTERED BASED DAY SUPPORT Program. The PROGRAM MANAGER shall also provide oversight and ensure the Implementation and documentation of Individual Person-Centered Plans, PCP Support Checklist, and any behavioral Person Centered Plans and programming. This position shall have knowledge of DBHDS, and the Office of Human Rights, and Medicaid Waiver Guidelines and Regulations. The PROGRAM MANAGER will supervise direct care personnel and assist with the oversight of staff scheduling to ensure adequate staff coverage to provide individual supports for the program and participate in hiring all CENTERED BASED DAY SUPPORT personnel. Additionally, the PROGRAM MANAGER is responsible for personnel management and evaluation of employees. This position shall require good oral and written communication skills and will provide Quality Assurance oversight for the support components of the Program. The PROGRAM MANAGER shall also serve as the QDDP. IT’S MY LIFE, LLC CENTERED BASED DAY SUPPORT shall secure a contractual agreement with a qualified individual to be the QDDP in the event that there are no personnel on staff to complete the duties and responsibilities of a QDDP. The PROGRAM MANAGER shall be on-call to provide support and oversight of crisis and emergency situations. The PROGRAM MANAGER will be reporting directly to/supervised by the Program Director.
