Overview\n \nThe Day Hab Program Activity Assistant (Instructor) aids in the implementation of clinical and individual programs. Assists in the daily implementation of DHSP, daily schedule of activities, and recommended plans. Provides support to persons served in day habilitation setting. Assists in case management responsibilities.\n \nResponsibilities\n\n* Provides instruction and skill building in the areas of Self Help, Sensorimotor, Communication, Social Development, Independent living, Behavior Development, Affective Development, and Wellness Development. \u2022 Assists in case management responsibilities, as directed. \u2022 Provides input for interdisciplinary team reviews. \u2022 Assists in the development of DHSP for persons served, including recommendations from professional and para-professional specialists. Maintains contact with family members, funding source, and other staff members, including members of the Interdisciplinary team at the Day Hab. \u2022 Implements clinical and daily plans for persons served. Provides recommendations or implements changes to ensure continuous improvement of programs. \u2022 Maintains documentation daily. Adheres to data collection and maintenance policies and guidelines. Ensures progress notes for persons served are completed according to guidelines. \u2022 Works directly with persons served individually and in groups. Works to develop skills for persons served. \u2022 Ensures that members are actively engaged in meaningful activities throughout the entire day, offering choices wherever possible. \u2022 Communicates with management regarding treatment and programs for persons served. \u2022 Participates or is actively involved with various teams, such as Case Management Team. \u2022 Supports members in crisis management using Positive Behavior Support (PBS) and Safety Care. \u2022 Assists with the training of staff, as needed. \u2022 Maintains physical upkeep of the program area. Monitors supplies and equipment needed. Ensures equipment is used, cleaned, and stored appropriately. Notifies managers of any needs identified and works to resolve those \u2022 Acts as advocate for persons served in all areas. Follows policies and procedures specific to program and Vinfen. Actively works toward community integration of persons served through community opportunities, volunteer opportunities, reverse integration, and the development of friendships. \u2022 Performs other related duties, as required. A valid driver's license must be presented at the time of employment. Incumbents must be at least 21 years of age, have maintained a valid US driver's license for at least six months and must be able to pass a driver's screening background check.\n \nPhysical Effort:Direct Care Roles - Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb as this is a very physically active position.\n \nMust be able to lift at least 25 pounds using proper lifting techniques or the use of a two-person lift.\n \nAbility to operate a computer and other office equipment such as a calculator, copier, and printer.\n \nAbility to sit, reach, climb stairs, and maneuver through narrow spaces or hallways.\n \nAbility to assist clients with tasks of daily living.\n \nAbility to speak, hear, and communicate with clients, staff, and external representatives.\n \nSpecific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.\n \nFor positions in day programs or group residences, the ability to assist in routine living activities including cleaning, meal preparation, vacuuming, shoveling, and grocery shopping.\n \nRequired Certifications/Trainings:CPR within 2 weeks of hire, First Aid within 2 weeks of hire, New Employee Training (NET), Safety Care within 90 days of hire\n \nPay Range\n \nUSD $22.00 - USD $22.00 /Hr.
