Job Purpose
We are seeking a compassionate and dedicated Day Center Aide to join our team at CSC PACE Center. This role will provide support to elderly or disabled individuals, ensuring their comfort and well-being in a safe and caring environment. You will be responsible for assisting with daily activities, personal hygiene, and providing companionship to enhance the quality of life for the clients we serve.
Duties and Responsibilities
Participant Care Responsibilities
Assist participants with activities of daily living (ADLs), including:
Ambulation and transfers
Toileting and incontinence care
Grooming and personal hygiene
Feeding and hydration assistance
Dressing assistance as needed
Obtain and document vital signs, weights, intake/output, and other observations as directed by licensed nursing staff.
Observe and promptly report changes in participant condition, behavior, or safety concerns to the nurse or interdisciplinary team.
Assist with restorative care, mobility exercises, and fall prevention measures under the direction of nursing staff.
Maintain participant dignity, privacy, and confidentiality at all times.
Support safe participant transportation within the center and assist with wheelchair safety and positioning.
