Occupational Therapy Assistant (COTA) Float – Full-Time | Lake Charles, LA | Assisted Living / Memory Care / Outpatient
Synergy Care is seeking a licensed Occupational Therapy Assistant (COTA) for a full-time position floating between two Assisted Living (AL), Memory Care, and outpatient clinic setting in Lake Charles, LA.
This role is ideal for a skilled nursing, geriatric, and outpatient-focused COTA who is passionate about rehabilitation, patient-centered care, and functional outcomes.
Key Responsibilities:
- Provide occupational therapy treatments to residents in AL, memory care, and outpatient settings
- Implement treatment plans established by the Occupational Therapist (OT)
- Focus on ADLs, functional mobility, cognitive therapy, and safety training
- Collaborate with an interdisciplinary rehab team
- Maintain accurate EMR documentation and compliance standards

Qualifications:
- Graduate of an accredited Occupational Therapy Assistant program (or graduating within 6 months)
- NBCOT certified (COTA/L)
- Current or eligible for Louisiana OTA license
- Experience in SNF, assisted living, memory care, or outpatient rehab preferred
