Occupational Therapy Assistant (COTA) – Full-Time | Lake Charles, LA | Assisted Living / Memory Care / Outpatient

Synergy Care is seeking a licensed Occupational Therapy Assistant (COTA) for a full-time position in an Assisted Living (AL), Memory Care, and outpatient clinic setting in Lake Charles, LA.

This role is ideal for a skilled nursing, geriatric, and outpatient-focused COTA who is passionate about rehabilitation, patient-centered care, and functional outcomes.

Key Responsibilities:

• Provide occupational therapy treatments to residents in AL, memory care, and outpatient settings
• Implement treatment plans established by the Occupational Therapist (OT)
• Focus on ADLs, functional mobility, cognitive therapy, and safety training
• Collaborate with an interdisciplinary rehab team
• Maintain accurate EMR documentation and compliance standards

Qualifications:

• Graduate of an accredited Occupational Therapy Assistant program (or graduating within 6 months)
• NBCOT certified (COTA/L)
• Current or eligible for Louisiana OTA license
• Experience in SNF, assisted living, memory care, or outpatient rehab preferred
