JOB SUMMARY:
- Under direction oversees the resident discharge process for the Sub-Acute Unit and performs the work involved in providing needed social assistance.
ESSENTIAL JOB FUNCTIONS:
- Participates and takes the lead in the utilization review process for the Sub-Acute Unit and prepares residents for discharge.
- Performs insurance reviews and issues the cut letters for Medicare clients.
- Participates with the staff relevant to diagnosis and the design of effective treatment plans.
- Provides social services to assist residents in the adjustment to their environment.
- Reviews cases of admissions and interviews patients to assure that certifications are in accordance with hospital criteria.
- Reviews records, data and information to assure that re-certifications are in accordance with needed extended care.
- Analyzes and evaluates data bearing on social and/or emotional problems residents are experiencing in properly adjusting to various situations.
- Prepares residents for discharge and utilizes family and community resources.
OTHER JOB DUTIES:
- Attends required in service and other training and development.
- Prepares records and reports.
- Does related work as required.
BASIC COMPETENCIES
EDUCATION:
- Possession of a Masters Degree in Social Work from an accredited facility.
- Possession of an LPN or RN license from the New Jersey Department of Health and Human Resources.
EXPERIENCE:
Four years of professional social casework experience or four years of experience in clinical
nursing which shall have included experience in the admission and discharge process.
