You'll keep services running smoothly, build real relationships with families and referral sources, and make sure nothing falls through the cracks. Key Responsibilities

Client Communication

• Maintain regular communication with patients and caregivers
• Address questions and resolve concerns in a timely manner

Service Coordination

• Coordinate services and ensure client needs are met
• Track client progress and follow up as needed
• Help ensure smooth day-to-day operations

Relationship Building

• Build strong relationships with families and referral sources

Qualifications

• Strong communication and interpersonal skills
• Organized, detail-oriented, and proactive
• Ability to manage multiple priorities
• Healthcare experience is a plus, but not required
