As a Physical Therapist, you will be responsible for assessing and treating patients to enhance their physical function and mobility. You will work alongside an interdisciplinary team to design and implement personalized treatment plans that promote independence and improve the quality of life for residents.

• Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
• Provide physical therapy interventions to improve strength, mobility, and functional abilities.
• Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications.
• Collaborate with the interdisciplinary team, including occupational therapists, nurses, and physicians, to coordinate care and optimize patient outcomes.
• Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes.
• Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies.
• Ensure that care plans align with regulatory requirements and best practices in physical therapy.
