As a Community Support Specialist, you will provide community-based mental health services to adult clients through assessment, diagnosis, treatment planning, service coordination, and intervention implementation. Working alongside clients, families, healthcare providers, and community partners, you will help create and implement individualized plans that foster resilience, recovery, skill development, and long-term success.

• Retention Incentive Bonus Program

  New Employees will be eligible to receive a $250 retention bonus upon reaching each of the following service milestones:

• 3 months of employment: $250 bonus
• 6 months of employment: $250 bonus
• 9 months of employment: $250 bonus
• 12 months of employment (1 year): $250 bonus

• To qualify for each payout, employees must be actively employed and in good standing on the applicable milestone date. Bonuses will be paid on the payroll cycle following the achievement of each milestone.

Key Responsibilities:

• Provide community-based mental health services that support client growth, recovery, and independence.
• Develop and implement individualized treatment plans in collaboration with clients, families, and other stakeholders.
• Utilize evidence-based and strengths-based approaches that promote empowerment, resilience, and skill development.
• Coordinate care with therapists, primary care providers, community organizations, and other service partners.
• Facilitate PSR group sessions that support client treatment plan goals and overall wellbeing.
• Facilitate ITCD group sessions as applicable to designated Community Support positions and client treatment goals.
• Build and maintain productive relationships with clients, support networks, and community stakeholders.
• Collaborate with interdisciplinary team members to ensure effective service delivery and continuity of care.
• Maintain timely, accurate, and compliant documentation of services, interventions, and client progress.
• Participate in ongoing training and professional development activities to strengthen clinical knowledge and service effectiveness.
