Community Support Counselor

Currently, we are seeking a part time Community Support Counselor to work at our Susquehanna Park Location.

Position Overview

As a Community Support Counselor, you will provide direct support and assistance to individuals in community settings, helping them access services, develop independent living skills, and connect with resources that support their well-being. Working under the guidance of the Program Director, you will play an important role in helping participants successfully live, learn, work, and socialize within their communities.

This position is ideal for someone who enjoys working directly with people, thrives in a community-based environment, and is passionate about helping others achieve greater independence and stability.

Key Responsibilities

Community-Based Support

• Provide individualized support services in participants' homes and throughout the community.
• Assist individuals in accessing healthcare, social services, employment supports, educational opportunities, and community resources.
• Help participants develop skills that promote independence, self-confidence, and community engagement.
• Encourage personal growth, self-advocacy, and meaningful participation in community life.

Participant Advocacy & Care Coordination

• Promote continuity of care by collaborating with team members and community partners.
• Support participants in identifying and achieving personal goals.
• Serve as a positive and supportive resource while encouraging recovery, wellness, and community integration.
• Monitor participant progress and communicate relevant updates to the Program Director and treatment team.

Crisis Support & Intervention

• Provide support during challenging situations and assist with crisis intervention when necessary.
• Respond appropriately to participant needs while maintaining a safe and supportive environment.

Qualifications

Required

• High School Diploma or GED.
• Valid driver's license with a satisfactory driving record.
• Strong verbal and written communication skills.
• Ability to work independently and responsibly in community settings.
• Ability to collaborate effectively as part of a multidisciplinary team.
• Commitment to providing compassionate, person-centered services.

Preferred

• Experience working in behavioral health, human services, social services, or community-based programs.
• Knowledge of community resources and support systems.
• Passion for helping individuals achieve greater independence and quality of life.

Work Environment

This community-based position may require:

• Frequent communication, active listening, and participant engagement
• Local travel throughout the service area
• Flexibility to respond to participant and program needs
• Physical activities including lifting, carrying, bending, kneeling, reaching, and other direct support activities
• Crisis intervention support during emergency situations
• Participation in scheduled emergency on-call services when required
• Reporting to work during emergencies, including severe weather conditions
