- Day-to-day travel can range from 5 minutes to 2 hours, with travel time varying by assignment.
- Mileage is compensated.
- The candidate may also need to travel to community events in Savannah, Augusta, or Macon approximately twice a year.
- Responsible for conducting home visits to high-risk members to complete a non-medical assessment to identify barriers impeding care such as home safety issues, transportation and other social determinants of health.
- Connecting with community partners and/or organizations to build partnerships that help to address the needs of our members.
- Coordinate, implement and participate in community events within the designated region but could include the entire Georgia market.
- Team/Individual is measured on the number of successful home visits completed per month (60% of the home visits will be successful).
- The role may require handling various challenges or crises, so individuals who are energetic, proactive, and enjoy solving complex problems in real-time will fit well.
- Provides support to members to connect them to known community and care resources in a cost- effective manner.
- Supports the coordination of community outreach resources available to members and promotes awareness of care/services.
- Serves as support for members on community and care resource inquiries and opportunities available to members.
- Supports all member related correspondence and educational materials to assist in the facilitation of a successful community connection.
- Documents and maintains all community resources to ensure standards of practice and policies are in accordance with health plan requirements.
- Provide assistance to the clinical team of nurses and social workers. Activities include, but are not limited to outreach, community education, informal guidance and member support.
- Conduct non-clinical general health assessments in order to refer members to appropriate care/services, resolve concerns on member’s behalf, and gather information for medical providers and staff working within the organization.
- Conduct non-medical assessments such as home safety, assessment of the community/environment resources, transportation, employment, and others to be able to refer to appropriate care/services, resolve concerns on member’s behalf, and gather information for medical providers in staff working within our organization.
- Conduct telephonic and/or in-person outreach to locate individuals and families in the community who are hard to reach.
- May make visits to individual homes and/or community organizations.
- Working Knowledge of Social Determinants of Health (SDOH) barriers
- Performs other duties as assigned.
- Complies with all policies and standards.
Key Requirements and Technology Experience:

- Knowledge of Community Health Practices: This includes understanding the health needs of the community, local health disparities, preventive health strategies, and social determinants of health.
- A strong foundation in these areas allows a CHW to effectively serve the community.
- Data Collection and Reporting: Ability to gather health-related data through surveys, interviews, or other methods and report on this data accurately.
- Familiarity with digital health tools, record-keeping systems, or data analysis can be crucial for tracking community health outcomes.
- Cultural Competency and Language Proficiency: Given that CHWs often work with diverse populations, understanding the cultural context and being able to communicate in the languages spoken by the community is a must.
- This could include fluency in specific languages or an understanding of cultural norms that affect health behaviors.
- High school diploma or equivalent.
- Requires 1 – 2 years of related experience.
- Must have Reliable Transportation/Willingness to travel, travel will be 90% of the role.
- Travel will entail the entire state of GA.
- Service Coordination Skills (knowledge of community resources and partners within designated region)
- Strong Communication Skills – verbal and written.
- Strong Organizational Skills / Time management.
